  • One of the country’s largest managed care organizations has selected the Medical HomeSM software to pilot a home monitoring based program with high utilization members with congestive heart failure (CHF). Members in the program, aged 55-65, were selected based on having multiple ER visits per year, among other factors. The goal of the program is to use in-home monitoring and automated reminders, alerts and reporting to members to increase their self-care actions, something that is demonstrated to deliver improved medical outcomes. The program will run at least twelve months, and will also be used to identify and manage operations management and scale issues for this type of program.
