Medical Home® solutions for Managed Care/Disease Management organizations focus on disease-specific initiatives (e.g. diabetes or congestive heart failure) and priority wellness initiatives (e.g. smoking cessation). Each program is developed to improve the member’s adherence to care plans. The member benefits by receiving wireless in-home monitoring devices such as blood pressure monitor, glucose meter, or peak flow meter that are easy to use and communicate directly with Medical Home® servers. This direct data feed from the member’s home or residence eliminates the need for error-prone self reporting of crucial care plan data (see table above).

Medical Home® incorporates a sophisticated alert system utilizing physician-defined parameters specific to the member. Alerts based on changes in weight, blood glucose and other vitals or events are sent to the member via an interactive voice response (IVR) system. The IVR notifies the member of the alert and reminds the member to follow predetermined physician directions or to contact their physician for further instructions. Reminders are also delivered through IVR, providing a cost-effective and customized approach to supporting the member’s self-care efforts.

Medical Home® builds on Managed Care/Disease Management organization's existing disease management and wellness programs, contributing to their ability to realize cost savings by reducing ER visits, inpatient length of stay, readmissions, and overall healthcare costs.
